Abstract

Abstract Date Presented 04/01/2022 This study used medical chart reviews to evaluate Lifestyle Redesign® OT services for hypertension and diabetes to improve health and well-being in a safety-net primary care setting. Patients showed significant improvements in clinical outcomes (systolic and diastolic blood pressure, HbA1c), as well as in patient activation and occupational performance. Findings support the promising role of OT as part of routine primary care for patients with chronic disease.
