A Rare Form of Ileocecal Intussusception with the Long-Compensated Course in an Adult

Abstract

Инвагинация это вариант кишечной непроходимости, обусловленный патологическим проперистальтическим внедрением проксимальной части кишечной трубки в просвет дистальной. Данная патология у взрослых является причиной кишечной непроходимости в 115 случаев и составляет 5 всех инвагинаций. Постановка диагноза в связи с отсутствием привычной для детей клинической триады (схваткообразная боль в животе, стул по типу малинового желе и пальпируемый инвагинат) затруднена. В статье приведено клиническое наблюдение, много лет протекавшей без явлений кишечной непроходимости, тонко-толстокишечной инвагинации у 38-летнего мужчины. Последний в двухлетнем возрасте оперирован по поводу острого перфоративного аппендицита, затем перенес релапаротомию с формированием концевой илеостомы, а позднее еще одну лапаротомию для ликвидации стомы и восстановления кишечной непрерывности. Оперирован в плановом порядке в 2019 г. в УЗ Минская областная клиническая больница с предоперационным диагнозом хроническая тонкотолстокишечная инвагинация, спаечная болезнь брюшной полости . Выявлен застарелый жизнеспособный инвагинат гигантской культи подвздошной кишки через баугиниеву заслонку в поперечно-ободочную кишку на фоне наложенного в детстве илеоасцендоанастомоза конец в бок . Выполнена резекция слепой кишки с инвагинатом. Послеоперационных осложнений не было. Intussusception is a type of bowel obstruction, in which a part of the intestine folds into the section immediately ahead of it due to pathological propulsive movement. Intussusception represents the cause of approximately 115 of bowel obstructions in adults and up to 5 of all intussusception cases in general population. Diagnosis of intussusception in adults is quite complicated because of the absence of the clinical triad (intermittent abdominal pain, rectal bleeding withred currant jelly, palpable mass), which is typical in children. The clinical case of the form of ileocecal intussusception, which has been existing in a 38-year-old man for a long time without bowel obstruction, is presented in the article. The patient was operated on at the age of 2 because of perforated appendicitis, then he underwent a laparotomy that was finished with end ileostomy, which was closed with the reconstruction of intestine tube later. In the present case, the patient was operated on in non- emergency order in Minsk Regional Clinic Hospital in 2019 due to the next diagnosis chronic ileo- colic intussusception, abdominal adhesions disease. There was revealed a viable invaginate of the giant stump through the Bauginium valve into the transverse colon on the background of end-to- side ileoascendoanastomosis applied in childhood. The resection of the cecum with invaginate was performed. There were no postoperative complications.
